All Iowa student athletes must have a physical exam performed by a licensed healthcare provider. The Iowa High School Athletic Association requires a preventive medical exam that goes beyond the basics to identify any potential health concerns.

To obtain the Pre-participation Form click here. Pages 1 - 2 including signatures by parent/legal guardian and student are to be completed prior to the exam.

Parental Consent

Parents should accompany their student athlete to the appointment. To initiate the exam, parental consent is required.

Insurance Coverage

This exam will be billed to insurance. Office visit co-pays apply. You are responsible for knowing your coverage.
