Security Breach Affecting Patients Who Received Care at a Similarly Named Facility in Missouri

Jefferson County Health Center is aware of a security breach affecting patients who received care at a similarly named facility in Missouri. Jefferson County Health Center of Fairfield, Iowa and its patients are not affected by, nor affiliated with, this Missouri facility. 

If you received a notice stating that your protected health information was breached in a security incident, we ask that you contact the facility listed in that letter.
