Sport Physical Nights at JCHC Clinics

JCHC Clinics is pleased to once again host sport physical nights for area student athletes. To help maintain appropriate social distancing levels, this year's physicals are split across two nights. 

On Tuesday, July 27, students with last names starting with A-L may come to JCHC Clinics for their physicals. Visit our event page for more details. 

On Thursday, July 29, students with last names starting with M-Z may come to JCHC Clinics for their physicals. Visit our event page for more details.

Both events run from 5:30 p.m. – 7:00 p.m.

The cost is $25 per physical, payable via cash or check. Participants must complete the Iowa Athletic Pre-Participation Physical Examination form as part of the physical. The form should be signed by the student's parent or legal guardian prior to examination. 

Scheduling for COVID-19 vaccinations will also be available during the event, though vaccines will not be administered.
