At Jefferson County Health Center, our Diabetes Education Program provides the tools, knowledge, and support you need to manage your condition with confidence. Whether you’re newly diagnosed or seeking better control, our team is here to guide you every step of the way.

Jefferson County Health Center has been Recognized by the American Diabetes Association for Quality Diabetes Self-Management Education* and Support. Our program is also certified by the State of Iowa.

Diabetes Classes

Our group class is designed to empower you to take charge of your diabetes and is taught every other month by a multidisciplinary team. The session covers all nine key areas of diabetes care and is perfect for those who are newly diagnosed or looking for a refresher. You only need to attend one session, as each class covers the same material. Classes are held in Conference Room C (use the McCreery Cancer Center entrance) from 12:30 to 5:00 p.m. and will be billed to insurance. Diabetes Community Group

The Jefferson County Health Center Diabetes Community Group is a welcoming space for individuals living with diabetes, as well as their families and caregivers. It offers an opportunity to connect with others who understand the daily challenges of managing diabetes, while learning helpful tips, sharing experiences, and receiving guidance from healthcare professionals. Whether you’re newly diagnosed or have been managing diabetes for years, this group empowers members with knowledge, encouragement, and a strong sense of community by fostering discussion, exchanging ideas, and providing mutual support for both those living with diabetes and their care partners. Free to attend, meetings take place from 10:30–11:30 a.m. in Conference Room C (use the McCreery Cancer Center entrance). View the Events tab to see upcoming dates.

MEET YOUR EDUCATOR

Amy Martin, RN, BSN, CDCES, serves as Jefferson County Health Center’s  Diabetes Care & Education Specialist, bringing both professional expertise and personal experience to her role. A Wayland, Iowa native, Amy earned her nursing degree from Culver-Stockton College/Blessing-Rieman College of Nursing and began her career at JCHC as a med-surg nurse in the old hospital. With more than 15 years of experience, she is nationally recognized as a Certified Diabetes Care and Education Specialist. Amy’s passion for helping others manage diabetes runs deep—her own daughter was diagnosed at age three, and because Amy knew the signs and symptoms, she was able to catch it early.

Amy provides personalized one-on-one education to help individuals understand their diabetes, prevent complications, manage medications, monitor glucose, and navigate the emotional side of living with a chronic condition. She also leads Diabetes Group Education Classes every other month, covering all nine aspects of self-management, and facilitates the monthly Diabetes Community Group for those living with diabetes and their caregivers.

Outside of work, Amy enjoys traveling, spending time with her husband, two children, and three dogs, and being outdoors—especially when Iowa weather cooperates. She is passionate about giving people hope and encouragement, reminding everyone living with diabetes that it’s a frustrating disease, but it’s not their fault.
